No damage except a new winter tire is trashed.

No tire shop to blame... Im honestly baffled.
I put them on, and made sure they were torqued and rechecked
later. No noise made, nothing un-to-do. At 30 km per hour the tire went zoom
and the nuts were all there within 20 ft of the center of excitement.

It was a near miss. The tire almost nailed a pedestrian.

I seriously wonder if this was someone doing something nasty.
I found everything except the center cap. If someone were to loosen
the nuts they wouldn't have been able to put the cap back on without
making noise.

The threads were all good on the nuts and bolts. I checked all the other
tires and the nuts were on good. I just don't know!

Wheel nuts that are properly installed don't back off in tight groups......like ever! I worked in a tire shop for 5 years and ran the service truck to several incidents where a wheel fell off. All wheel nuts were never located and you found yours all within a short distance.

You may have been the victim of vandalism/mischief. Think about getting a set of wheel locks that have a good quality key socket like McGuard. 40.00-50.00 is a good investment to deter this kind of thing.

Agreed, mine almost came off on my way home from Whistler. If I didn't have a passenger in the rear I'm sure it would have. The only explanation was that the wheels had been loosened off while I was up the mountain. Whether it's simple vandalism, or someone trying to steal your new tires (either were a possibility for me as well) I would and did invest in some wheel locks.

I have seen it a few times did you clean the mounting surfaces, did you torque after 50km. Also did you lube the threads of the studs?

It's all second guessing today. I will pick up a new tire and look at some locks. It sucks the tire got a cut in the sidewall. And I got a cool picture.

My Story...

1972 Cutlass with an aftermarket cam and headers (many years ago). So I got a ticket to repair a faulty tail light and I knew I had a couple bald tires so before I went for the inspection I replaced two tires with tires and rims from my friends. We measured everything and it was all good, spent the afternoon working on the car to get it up to snuff. Well on the way to the cop shop I heard a squeeking noise, I pulled over and tried to shake the tires by hand to see if they were loose. All seemed perfect so I proceeded to drive to my destination. Next thing I knew my car jumped into the air, the car came down, my head hit the roof before the car slid to the ditch... I saw my left front tire rolling down the road. I ended up having a huge dent behind the wheel well that looked terrible and caused the door to not work easily. When I got the the cop shop they didn't even look at my car, I was so pissed.

if the tires are loosening you need to jack it up to test
never go by what looks to fit
take them to a tire expert
I would also suggest lock nuts

I've been installing tires since I was a kid. Very meticulous. I was just at the tire shop and the 2 owners said it looked suspicious since there was no wear around the holes being aluminum. Said more often than not they wear oval from the rubbing before the tire goes suggesting the nuts weren't loose for long.
He said 99 times out of a 100 there is some damage to the rim. He suggested some locks as well

I had a tire come off of my bug once (front drivers), and yeah, I thought for sure it was vandalism, to the point that I even mentioned it to ICBC. Kinda freaky driving down the road and seeing your tire roll on past you..
