Big vibration at idle when in drive or neutral.

Does your Mitsubishi L300 make a strange noise? Need wheel alignment specs?

RPM Too low ? What is the RPM ? If low, just try, at first, to pull gently (a little) the choke once started and check the vibrations

A little more information might help- as Lapprentis asks, what is your idle speed? It should usually be 750rpm when warm, and you should set it to 1000rpm while the engine is warming up.

Does the vibration happen when the engine is cold or hot? Does it improve when the engine is hot?

Is there any smoke from the exhaust?

Does not make a difference when hot or cold. Rpm at 250 when in drive position. Goes back to 750 when in park.
I have no choke on my van.

Vibrates even more when in 4wd and in drive not on park. Looks like the steering wheel will come off.

Wait, choke? It's a gasoline engine? Sorry, I presumed it was a diesel.

Gasoline engine idle speed should be closer to 850rpm. 250rpm in drive is extremely low. Does the shaking stop if you use the accelerator to increase the idle speed?

I'd initially suspect a dirty throttle body, particularly the idle speed control.

Worst case scenario is a faulty torque converter lockup clutch that's staying engaged. When driving, does it shift smoothly or is it always a little jerky?

Its a diesel. Do you mean a manual choke or in the engine? Dont have a manual choke on the van.

Yes, Its a diesel. It has that auto idle adjustment connected to the IP. It does not have that usual throttle cable most L300 have.

Basically.

First.
We tried to adjust the idle at Park to what it should be. 750+-50Rpm.
When we drop the shifter into Drive, it drops between 250 to 500Rpm. Shakes like the mother load. not good at all.

Second,
we dropped it to Drive then adjusted the idle to 750+-50Rpm, but when its back into Park she idles to 1500Rpm.

Then, we tried to adjust it partway but no differences.
When it's adjusted to that 1500Rpm, of course when she is engaged into Drive she wants to go go go... Not good!

It does it cold or warm... all the time.

Checked sensors, fuel, fuel lines, glow plugs, glow plug relay, I've never had this issue before!

Ah. Automatic cold idle speed control is unusual, so I'm guessing it's a later model, like a '94 or so.

If the idle speed is fairly consistent (and it sounds like it is) then my best guesses are either a sticking governor inside the injection pump, an incredibly worn engine, or a partially locked-up torque converter.

Sticky governor: Not much you can do about this one without removing the injection pump. I would replace the fuel filter, check the banjo fitting where fuel comes into the IP (there's sometimes a tiny fuel screen in there that can get clogged), and maybe run some seafoam through it.

Worn engine: You'd notice blueish smoke out the exhaust, lots of oil leaks, and the engine would be very slow. Not much you can do about this one either, but to get to the point where it didn't have enough power to keep itself idling it would have to be fairly bad.

Failing torque converter lockup: I would expect the transmission to shift harshly when driving. An auto transmission shop should be able to diagnose it, and a transmission fluid flush and change might fix it.

Just an observation: It's a little tricky to diagnose a fault when you have to guess what kind of engine it is. Jimmy, consider updating the vehicle information in your profile, or start each query with "1994 Super Exceed Diesel" or whatever it is.
