Ideally you'd add or remove shims until you have the clearance the manual says, but I've seen a few vans with no shims at all so it's probably ok.

Yup, they get stuck after a few years. An oil filter wrench with a strip of rubber to protect the chrome sometimes works too.

Is this a diesel engine? If so, you have twin fan belts, and you need to replace them both at the same time with a matched pair of belts from the same batch. Blue smoke does sound like worn piston rings. You'll need to do a compression test: 440psi when cranking is ideal, the lowest limit is 320psi, with no more than 20psi difference between any two cylinders.
If the compression is ok it could be failed valve stem seals, or a slightly leaky turbo.

When it's doing this, open the engine cover and check that the throttle lever is resting on the idle speed screw like it should be. As Steven says, it might be your AC idle-up solenoid holding the lever back (or binding in the accelerator cable, or the kickdown cable, or... or... or... lots of thing...

I just figured that the position of the shifter lever tells me when it's in 4WD. (Whereas reverse needs a switch to turn on the reverse lights). As long as the hub lock light works I'm happy.

Nice! I thought about doing something similar for the 4WD dash light which is always a bit intermittent, but it makes far more sense for the reverse switch.
