I think Old Man Emu out of Australia makes them. Probably alot of cash to bring in though. I know there are custom spring shops out there too that will design them to your specs. A set of custom progressives would be awesome! Have you tried fitting your '99 with 33's? I can fit 33x10.5 on stock offs...

Google toyota sequoia spacers in pajero or montero. I forget what years fit truck but I think it was most if not all. 4x4wire and other forums are great sources of info.
I believe they only come in 1" but there may be more options available.

Also check the Throttle Position Sensor.
Lots of posts, easy to tune up, makes a huge difference how the vehicle shifts. I do mine yearly as it starts to waver.

Looks pretty good. You could modify some sort of plastic knob to fit over the wing nut with the edges trimmed. You could probably find a stock unit at a wrecker too.

I've had that problem with a stuck EGR valve. The noise not so much, but every other symptom.
I've also had similar issues when an attendant filled 3/4 tank with gas instead of diesel, regardless as me asking for diesel and there being two diesel only stickers on the door and gas cap.

As a tip for Albertans, Co-operators insurance will offer full collision and comprehensive on your van. You just need an insurance inspection report. (I think any vehicle more than 10 years old requires that with most insurance companys)
