a rear facing car seat is either secured by the lap portion of the seatbelt as per manufacturers instruction or the anchor tethers that come out each side to replace the use of the seatbelt (only if you have manufacture installed anchor points).
it does not use a tether at the back of the head in a rear facing position. when you turn it around to be forward facing then you will use the tether at the back of the head to go over the back of the seat and down to an anchor point.
