I was able to find a used Espar at a reasonable price and I'm very happy with it's results. The fire chamber did foul up on me a little while after I got it however parts and new gaskets were easy to find at local shops that deal with heavy equipment. If you're still looking for answers, I put a hydronic coolant heater onto my L400 (diesel) and i just teed into one of the rubber fuel lines in between the hard line sections along the body rails underneath roughly under the sliding door. Other vehicles that used the 6G72 engine were 1987-2000 Dodge Caravans among plenty of others... no one then or now is feeding their Caravan premium fuel and don't have the compression ratios to require it.

About the largest 'standard' replacement tires with no lift are 235/75R15... A good tire shop will be able to easily find most light truck varieties and put them on (aka Costco will get grumbly with you that they aren't 'stock' size)

I have a set of BF Goodrich All-terrain T/A KO2 235/75R15 on my 1995 L400 with the stock delica rims and they are great on there (no lift on the van either). You shouldn't run into any problems at all
