I read in a Pajero thread that cylinder head cracks tend to happen to the 4m40 over time.

Would a repair shop be able to do a pressure test to tell me if it indeed is the cylinder head and not just the gasket?

I wonder if it might be possible to install one on an older Delica model which didn't come with one.

Might not be worth the hassle though, if the step isn't all that useful.

I bought a ladder from Japan, not a Terzo but probably they all hook up the same way. Mine ends with just two bolts at the top, I am wondering what the two bolts connect to normally.
